Adventist Health Columbia Gorge Family Medicine has an outstanding opportunity for an Advanced Practice Provider. Family Medicine is a designated NHSC (National Health Service Corps) service site allowing for loan repayment options, as well as a Rural Health site offering Oregon income tax credit. Our team welcomes new graduates and experienced doctors alike, to work with a cohesive group of APPs, physicians, nurses and medical assistants who are very passionate about evidence-based whole-person care. We are a designated 5 STAR Primary Care Medical Home with integrated Behavioral Health, on-site diabetes education and EPIC EMR.

Education:

• Graduate from an accredited NP or PA program
• PA must have master's degree

Licensure/Certification:

• Board-eligible or board-certified in Family Medicine
• Unrestricted license and DEA to practice medicine in the state of practice
• BLS
