details found on our College to County website. What Does the
Prevention and Health Promotion Unit Do? The Chronic Disease
Prevention & Health Promotion (CDPHP) Unit works to reduce
Multnomah County's leading causes of death by creating a community
context that promotes health and protects against preventable
injury and chronic disease. The program strives to enhance health
outcomes and diminish health disparities by targeting social
determinants of health and promoting initiatives that support
communication campaigns, policy changes, and systemic and
environmental modifications. CDPHP works throughout individuals’
lives and within communities to prevent, reduce, and manage chronic
diseases, with the goal of decreasing preventable deaths and
injuries. CDPHP houses the Tobacco Control and Prevention Program
(TCPP), Community & Adolescent Health (CAH), and the Racial and
Ethnic Approaches to Community Health (REACH). Staff, including
community health specialists, will work with communities and
coalitions to support population health across the lifespan by
addressing and increasing access to chronic disease prevention
strategies and community violence and injury prevention. These
efforts will result in policy, system, and environmental change
strategies that improve community health by addressing the impacts
of racism and social determinants of health. Key components of this
work include public health modernization, community-clinical
linkage programming, and community partnerships through shared risk
and protective factors. These initiatives work together to remove
barriers and improve the overall health of Multnomah County
neighborhoods. Program Goal: To reduce health disparities and
Multnomah County’s leading causes of death, the program will
improve health outcomes by addressing social determinants of
health, creating and fostering social cohesion, leveraging
community wisdom, and promoting a community context that supports
health and prevents injury and chronic disease. Prevention & Health
Promotion Strategies and Activities: Taking upstream approaches to
creating and conducting community health assessments to identify
priority areas for interventions, including policy, systems and
environmental changes. Develop and implement evidence-based
interventions to address social determinants of health and promote
healthy behaviors. Identify and increase awareness through outreach
and engagement policy changes that support chronic disease
prevention and health promotion. Conduct communication campaigns to
raise awareness of chronic disease prevention and health promotion.
Strengthen community-clinical linkage through systems changes to
promote utilization of health and community resources by expanding
the community health worker model and improving care coordination.
Ground culturally tailored strategies in community based and
evidence based approaches that mesh public health interventions and
collaborate and engage communities. Internship Description &
Responsibilities: Contribute to the planning, set-up, community
outreach, and implementation of health events within Prevention and
Health Promotion Unit, including TARA, CPTED/Walk audits, REACHing
Farmers Markets and PRIDE NW. Support on-site outreach, engagement,
and distribution for Community Supported Agriculture Programming
and Food & Vegetable Voucher/CSA Rx programs. Participate in
partnership activities with ACHIEVE Coalition Partners, such as
PlayGrowLearn. Assist with researching and updating resources
related to social determinants of health, including food,
transportation, health care, and employment. Engage in intern
cohort meetings, training, and job shadowing opportunities.
Complete other tasks and special projects as assigned. Learning
Outcomes: Gain practical experience in local public health and
government operations. Enhance skills in information gathering,
analysis, and resource development related to social determinants
of health. Improve public health communication abilities, including
effectively conveying evidence-based information and community
insights. Minimum Qualifications and Education : Candidates should
have completed or enrolled in a college/university academic
institution. Preferably a Public Health Major Promote a culture of
