Job Description:
Legacy Health, a prestigious non-profit health system spanning
across Oregon and Washington, is seeking a highly skilled and
knowledgeable RN Disease Specific Program Coordinator. This role is
fundamental in leading and coordinating our healthcare services
tailored to specific diseases at multiple facilities within our
network. The coordinator will play a crucial role in program
planning, staff management, patient care, and ensuring compliance
with healthcare regulations and standards. This position offers a

solutions in disease-specific healthcare programs. Develop,
implement, and evaluate disease-specific programs aligning with
organizational objectives and compliant with medical practice.
Coordinate with healthcare teams to ensure seamless delivery of
care and services across disciplines and facilities. Monitor
patient outcomes and implement improvements in the disease-specific
programs based on evidence-based practices. Provide training and
support to staff in delivering specialized care related to specific
diseases. Engage in continuous professional development of the team
and the program, staying updated with advancements in disease
management. Lead interdisciplinary meetings and collaborate with
healthcare professionals to review and strategize on patient care
plans. Maintain documentation and compliance with all applicable
laws, accreditation standards, and regulations. Strong leadership
and organizational skills Excellent communication and interpersonal
skills In-depth knowledge of disease management and healthcare
practices Proficiency in patient care and program management
software Ability to work collaboratively in a multidisciplinary

Insurance 401k Active Registered Nurse (RN) license in the state of
practice Certification in disease-specific areas related to the
program (e.g., Certified Diabetes Educator, Oncology Certified
Nurse) may be highly desirable or required based on the disease
focus.
