Job Description:
The Social Work Supervisor provides oversight to social work for
completion of comprehensive biopsychosocial/spiritual assessment,
diagnosis and/or treatment/interventions/advocacy of participants
and their support systems. Acts as the geriatric/medical social
work expert and clinical supervisor to social workers (LCSW, CSWA,
SSA) both within their immediate social work teams and PMG at Home.
The Supervisor ensures that Social Work care planning is
evidence-informed person-centered across the care continuum in
collaboration with the Inter-Disciplinary Team (IDT). This position
requires travel between multiple sites. The Supervisor performs
duties in a manner that respects participant self-determination,
reduces health disparities/barriers to care, and adheres to the
NASW code of ethics, Standards of Practice (SOP), and mission. The
Supervisor also performs the Licensed Clinical Social Worker (LCSW)
duties as part of role. Independently provides comprehensive
biopsychosocial/spiritual assessment, diagnosis and/or
treatment/interventions/advocacy of patients and their support
systems. The LCSW incorporates concepts and theories associated
with life span development, neurobiology, and behavioral change.
The LCSW will collaborate with patient and support system to
develop and implement evidence-informed person-centered care plans
across the care continuum in collaboration with the
Inter-Disciplinary Team (IDT). The LCSW shall perform duties in a
manner that respects patient self-determination that reduces health
disparities/barriers to care, and adheres to the NASW code of
ethics, Standards of Practice (SOP), and mission. Providence

Social Work: from a CSWE Accredited School of Social Work.
Education to meet certification, license or registration
requirement. Oregon Clinical Social Worker License upon hire. 8
years of relevant post MSW clinical Social Work Experience. 5 years
of social work experience with the medically fragile adults. 2
years of Supervisory experience. Preferred qualifications:
Coursework/Training: Advanced training. 2 years of experience as a
subject matter expert, clinical licensure supervisor, trainer, or

minimum and the maximum wage rates on the wage range for this
position. The successful candidates placement on the wage range for
this position will be determined based upon relevant job experience
and other applicable factors. These amounts are the base pay range;
additional compensation may be available for this role, such as
shift differentials, standby/on-call, overtime, premiums, extra
