Manager RN at Providence Portland Medical Center. This is a
Full-Time (1.0 FTE), Day Shift position. RN Care Managers, in the
acute care setting, provide a variety of services to appropriately
help progress patients through the continuum of care. Key services
include screening for post-acute needs; transition planning; care
coordination; education; community care referrals; care planning in
accordance with regulatory requirements; collaboration with the
continuum care team; ongoing monitoring of plan effectiveness and
reassessment of the need for Care Management (CM) services.

degree/diploma required upon hire. Bachelors degree in Nursing or
higher within 3 years of hire. Upon hire: Oregon Registered Nurse
License 2 years acute care hospital or community based experience.
1 year experience with electronic medical records or documentation
systems. 1 year home health, mental health, substance use, hospice,
utilization review, discharge planning or care management
experience Preferred Qualifications: Bachelors degree in nursing

Posted are the minimum and the maximum wage rates on the wage range
for this position. The successful candidates placement on the wage
range for this position will be determined based upon relevant job
experience and other applicable factors. These amounts are the base
pay range; additional compensation may be available for this role,
such as shift differentials, standby/on-call, overtime, premiums,
extra shift incentives, or bonus opportunities. Providence offers a
